Preheat oven to 175°C and lightly grease a standard loaf pan.

In a large mixing bowl, thoroughly blend ground beef, breadcrumbs, Parmesan cheese, milk, salt, and pepper until well incorporated.

Transfer the meat blend to the prepared loaf pan, pressing it firmly and forming a shallow channel down the center.

Heat a skillet over medium heat. Sauté diced onion, green bell pepper, and mushrooms until softened and aromatic, then remove from heat.

Spread sautéed vegetables evenly into the center well of the meatloaf. Distribute shredded provolone cheese over the top.

Place in the preheated oven and bake for 60 minutes, or until internal temperature reaches 70°C and meat is thoroughly cooked through.

Remove from the oven and allow to rest for 10 minutes before slicing and serving.
