Combine ground beef, breadcrumbs, egg, garlic powder, onion powder, salt, and pepper in a mixing bowl. Mix until just incorporated.

Form the mixture into 4 to 6 oval-shaped patties with uniform thickness.

Sear patties in a large skillet over medium heat for 2–3 minutes per side until browned but not fully cooked. Transfer to the prepared baking dish.

In a bowl, whisk together cream of mushroom soup, milk, and Worcestershire sauce until smooth.

Pour gravy evenly over patties. Top with sliced onions and mushrooms if using.

Cover the dish with foil and bake for 30–35 minutes.

Remove foil and continue baking for an additional 10 minutes, until the sauce is bubbling and hamburger steaks are cooked through.
